1) Your name and complete address,

2) Your voice telephone and fax numbers, and your email address,

3) Your tax ID number (Brazil and Mexico only,)

4) Permit documentation (if you are ordering mist nets,) and, of course,

5) Your payment.

PAYMENT TERMS. Payment must be made in advance, in US Dollars only. Payment by credit card is quite convenient. We accept Visa, MasterCard, American Express, and Discover cards, and Pay Pal. If you prefer, however, you can pay in advance in U.S. funds by bank check, or wire to our account. Bank checks must be in US funds, payable in a U.S. city through a U.S. bank. The correct type of check will have the name of the U.S. city printed on the front. (Your banker can assist you with this.) If you send a wire, please arrange to send enough to cover all bank wire fees.

FOR NET ORDERS ONLY. Your order will not be shipped unless our permit conditions are satisfied. Please send your permit information for our approval in advance of sending any payment. If your country does not issue banding, ringing or bat research permits, please supply BOTH of the following:

a) A brief description of your project, AND

b) A letter of recommendation from someone outside your organization who can tell us about your training in the use of mist nets. A U.S. referral is best.

Vampire bat research requires a special questionnaire which is available in Spanish. Your detailed responses to our questions will be submitted to a qualified referee who will evaluate your responses. This evaluation can take a few weeks. For vampire bat work, please ask us for the questionnaire.
